Host 'Chinook' Application  Form  

Thank you for your interest in hosting Chinook during his Global Heart Warming Tour.   

We ask that you fill out the form below to the best of your ability.  The information in the form will be kept confidential.   Applications are reviewed by North Country Teddy Bears Administration for suitability and accessibility.   Applicants will be contacted regarding the outcome of the review.  

Successful applicants will receive further instructions in a Host Information Package.  This package contains arrival and departure dates, shipping and reimbursement instructions, press release  and contact information, story templates,  photographic specifications,  and other pertinent information to help make Chinook's stay with you successful.

Successful applicants agree to have their Name and Country published on this site for the further promotion of Chinook's Tour.  Other information will only be posted pending formal approval by the Host.

All shipping expenses for Chinook are reimbursed to the Host by North Country Teddy Bears upon the NCTB Tour Coordinator receiving an official postal receipt for successful shipment.  Again, thank you for your interest!
